Essential Collective Care
At Essential Health – Raleigh, our objective is to help you reach the highest level of well-being possible.
Our primary care programs are not designed to maintain the status quo, but to address the underlying issues that are preventing you from optimizing your health.
Essential Health – Raleigh offers two lower cost monthly primary care subscription options, Essential Collective Care and Total Collective Care. These programs are perfect for patients seeking a whole-body approach to the management of primary care concerns, medications, and sick visits, but do not require full concierge-style primary care benefits.
Essential Collective Care (ECC) Benefits:
- Access to provider visits during business hours via portal, phone, in-person, or telemedicine
- All lab reviews, as needed
- Management of primary care medications and concerns
- Follow-up care and same-day sick visits with available provider
- Coverage of select ancillary services pertaining to illness (i.e. EKG, flu testing, covid testing, urine dip testing, etc.)
$150/month
Total Collective Care (TCC) Benefits:
Total Collective Care includes all features of ECC, in addition to:
- Management of complex functional and lifestyle medications, such as hormones and peptides
- Longer and more in-depth quarterly visits
- Quarterly functional lab panels, with additional testing annually
- Additional discount on supplements via our online dispensary, FullScript®
$325/month
Both programs are personalized to your needs and goals and include more frequent touchpoints with your provider than the industry standard of once a year. These programs also provide evidence-based, cutting-edge therapies along with guidance from your provider to promote optimal health and longevity. ECC and TCC are offered at a lower monthly cost than our flagship Essential Primary Care concierge-style program, with the same personalized attention you need to feel your best.
We welcome new and existing Essential Health patients to join ECC or TCC. Please note that a Collective Care Wellness consultation ($1,100) is required for new Essential Health patients beginning either primary care program.

Nikki Fowler, FNP-C is a Family Nurse Practitioner at Essential Health – Raleigh. Nikki designed Essential Collective Care and Total Collective Care to provide patients with personalized, comprehensive support at a lowered monthly cost for those who may not need a full concierge-style program. She is thrilled to welcome current and new patients into these programs at the Raleigh office.

Essential Health services are provided on a cash basis. We do not accept, nor do we bill, health insurance. All additional labs and advanced diagnostics outside of the ECC or TCC memberships are to be paid a ’la carte.
