Essential Health Assessment
Every new adult member begins with an Essential Health Assessment (EHA). This is an in-depth, physician-led evaluation designed to establish a comprehensive baseline of your health, identify risks and opportunities early, and create a personalized strategy for the years ahead.
Your assessment includes comprehensive laboratory testing of more than 100 biomarkers, DEXA bone density and advanced body composition, VO₂ max fitness testing, carotid ultrasound and plaque assessment, EKG, grip strength, pulmonary function testing, hearing and cognitive assessments, a comprehensive medical history and physical examination, and more than three hours of dedicated physician time.
Dr. O’Brien will bring these findings together into a clear, personalized health and longevity plan, including priorities, recommendations, and measurable goals for the year ahead.
Adult EHA (ages 31+): $2,950
Young Adult EHA (ages 19-30): $1,950
An annual repeat EHA is included with your monthly primary care membership. An EHA can be completed as a one-time assessment outside of a primary care membership.

Functional Medicine
Functional medicine is not a separate service at Essential Health Sarasota—it is woven into how we practice primary care. In addition to diagnosing and treating immediate medical concerns, we look beyond isolated symptoms to identify the underlying causes and contributing factors that may be affecting your health, including nutrition, lifestyle, sleep, stress, genetics, hormones, gut health, and environmental exposures. This whole-person approach can help address persistent health concerns while supporting prevention, long-term vitality, and overall well-being.
Women’s Health
Women’s health is integrated into our comprehensive primary care approach, with personalized support for menstrual and hormonal concerns, fertility planning, perimenopause, menopause, thyroid and metabolic health, bone health, and healthy aging. Treatment may include lifestyle and nutritional interventions, targeted supplementation, nonhormonal therapies, peptide therapy, or individualized bioidentical hormone replacement therapy when appropriate.
Men’s Health
Men’s health at Essential Health - Sarasota extends well beyond testosterone. Dr. O’Brien provides a comprehensive, data-driven approach to cardiovascular and metabolic risk, weight and body composition, energy, sleep, physical performance, and healthy aging.
Symptoms such as fatigue, low libido, reduced strength, weight gain, or poor recovery can have many contributing factors. We evaluate the complete picture, including hormones, nutrition, sleep, stress, medications, metabolic health, and lifestyle before developing a personalized plan. Treatment may include lifestyle and nutritional interventions, weight management, targeted supplementation, peptide therapy, medications, or medically supervised hormone replacement therapy when clinically appropriate.
Pediatric Care
Pediatric primary care is beneficial for a child needing a “deeper dive” into their health and ongoing primary care support. Our Primary Care Child Membership is designed for children and adolescents under 18 who may be struggling with attention, hyperactivity, malabsorption, gastrointestinal distress, inflammation, skin issues (rashes, acne), hormonal concerns such as menstruation or puberty concerns, possible environmental toxin exposure, mental health, or general wellness. Please note, a parent or guardian Adult Membership is required for enrollment in a Child Membership.
Peptide Therapy
Peptide therapy is one of several advanced treatment options Dr. O’Brien may incorporate into a personalized care plan for Primary Care Members. Recommendations are based on your medical history, laboratory findings, health goals, and the potential benefits and risks of treatment—not a one-size-fits-all protocol.
When clinically appropriate, carefully sourced peptide therapies may be used to support specific goals such as weight management, metabolic health, recovery, physical performance, and healthy aging. All protocols are medically supervised, individualized, and monitored over time.
Personalized Supplementation
Supplements should have a clear purpose. We consider your symptoms, medical history, nutrition, medications, and laboratory findings to determine which supplements may be beneficial for you. Recommendations emphasize quality, safety, potential interactions, and periodic reassessment so that your regimen remains effective and appropriate over time.
Primary Care Members receive access to carefully vetted, professional-grade supplements at practice cost, which provides significant savings compared with typical retail pricing.
Hormone Replacement Therapy
If needed, we utilize Bioidentical Hormone Replacement Therapy (BHRT) as a part of your Primary Care Adult Membership to help men and women restore hormone balance and improve energy, mood, and overall wellness. Our personalized HRT solutions address symptoms like fatigue, brain fog, weight changes, and low libido. For women, BHRT provides safe and effective menopause support, while for men, hormone therapy can help improve vitality and focus.
Regenerative Medicine
Essential Health Sarasota offers personalized regenerative medicine consultations and select exosome and stem cell therapies for appropriate adults with orthopedic, pain-management, or wound-care concerns. Following a comprehensive evaluation, we review conventional and regenerative options, the available evidence, potential risks and benefits, alternatives, and the investigational status of treatment so patients can make informed decisions.

Frequently Asked Questions
How is Essential Health Sarasota different from a typical concierge practice?
Many concierge practices primarily add greater access and convenience to a traditional, insurance-based primary care model. Essential Health Sarasota was designed differently. Dr. O’Brien limits his practice to approximately 200 patients and does not bill insurance for physician visits, allowing him to provide unlimited, unhurried appointments, same- or next-day availability when appropriate, direct communication, and 24/7 access for urgent concerns.
The difference extends beyond accessibility. Functional and longevity medicine are woven into comprehensive primary care rather than offered as separate services. Adult and Young Adult Members receive an annual Essential Health Assessment, comprehensive laboratory reassessments approximately every four months, and a personalized health strategy that evolves throughout the year. The result is a more proactive relationship focused not only on treating illness, but also on identifying risks early, preventing disease, and optimizing long-term health.
Is there a long-term contract?
No. Membership is billed month to month and can be canceled at any time without penalty.
Can I still use insurance for imaging, medications, specialists, or hospital care?
Yes. Essential Health - Sarasota does not replace health insurance. Insurance continues to work normally for outside services such as imaging, medications, specialists, emergency care, hospital care, and procedures. You are also able to keep your insurance-based primary care physician, though we encourage members to use Essential Health - Sarasota as their primary care home.
Can I use HSA or FSA funds for membership?
Some patients may be able to use HSA or FSA funds for certain membership fees, depending on their plan and current tax rules. Please confirm with your HSA/FSA administrator or tax advisor.
