Essential Health – Raleigh is pleased to announce the launch of two new primary care programs, Essential Collective Care and Total Collective Care! These distinct, direct primary care memberships are designed for patients ages 5 and up who are seeking a whole-body approach to the management of primary care concerns, medications, and sick visits, but do not require full concierge-style Essential Primary Care benefits.
Essential Collective Care and Total Collective Care are spearheaded by Nikki Fowler, FNP-C, along with the support of the wider Essential Health – Raleigh provider team.
“Essential Collective Care is our base program focusing on basic primary care and preventative needs, whereas Total Collective Care offers more comprehensive care and is tailored for patients with hormone and/or peptide needs,” says Fowler. “Both programs include a comprehensive annual work-up, quarterly check-ins, and support for any acute needs.”
Essential Collective Care benefits include:
- Access to provider visits during business hours via portal, phone, in-person, or telemedicine
- All lab reviews, as needed
- Management of primary care medications and concerns
- Follow-up care and same-day sick visits with available provider
- Coverage of select ancillary services pertaining to illness (i.e. EKG, flu testing, covid testing, urine dip testing, etc.)
Total Collective Care includes all features of Essential Collective Care, in addition to:
- Management of complex functional and lifestyle medications, such as hormones and peptides
- Longer and more in-depth quarterly visits
- Quarterly functional lab panels, with additional testing annually
- Additional discount on supplements via our online dispensary
How to Get Started & What to Expect
To see if Essential Collective Care or Total Collective Care could be right for you, complete a Meet & Greet (preferably in-person) with a Collective Care provider. For new patients, these memberships do not require or include an Essential Health Assessment, but instead require a specialized Wellness Consultation, called Collective Care Wellness. Once you are enrolled, you may contact the office during business hours for same-day sick visits or follow up care as needed with your provider. With monthly membership costs starting as low as $150/month, you can enjoy comprehensive, personalized support from a functional medicine provider committed to your whole-person health.
